ENDODONTIC REFERRAL FORM

Dr. Mary Dabuleanu, Dr. Gevik Malkhassian, Dr. Payman Mehrvarzfar and Dr. Pooneh Mohebbi are our Canadian Board Certified Endodontists. They are dedicated to the wellness and comfort of their patients, and they will do their best to make your root canal experience a pleasant one. They specialize in both the non-surgical as well as surgical treatment of initial root canal disease, and in the repair of previously root canal treated teeth that have persistent disease. All endodontists are current or past clinical demonstrators at the University of Toronto.  Dr. Gevik Malkhassian is a current Assistant Professor at the University of Toronto.   Dr. Mary Dabuleanu is available Monday to Friday to assist your patients.  Dr. Pooneh Mohebbi is available on Wednesdays and Saturdays to see your patients.   Dr. Payman Mehrvarzfar is available Fridays. Collectively, they will do their best to accommodate your patients for emergency treatment the same day, Monday through Saturday, and they will welcome your patients in a friendly environment that is focused on addressing their concerns or discomfort, both expediently and empathetically. Our office offers different sedation modalities.  We offer minimal sedation using oral or nitrous oxide sedation and moderate intravenous conscious sedation.  Dr. Gino Gizzarelli is our visiting dental anesthetist and will provide general anesthesia if necessary, for optimal patient comfort and recovery. Each case will be assessed independently.

Once treatment is complete as per the referral, all patients are immediately referred back to your office for ongoing dental care. We ask that referring dentists complete a referral form.

 

We are conveniently located on the North-West corner of Yonge and Finch and are easily accessible via subway.  Parking is available both underground in the Xerox tower as well as in the Green P parking lots across from our office.
